With the convenience of our drop-off service, it is imperative that we have the ability to communicate with you. Once our doctors make an initial assessment of your pet’s problem or complete all requested services, we will call you to discuss the findings. If your pet is sick, we will assess the problem, formulate a treatment plan and call you with the details.
Please understand that we will make every attempt to give you an estimate of all expected charges. All charges incurred will be your responsibility at the time of discharge.

I understand all anesthesia & surgery involves some potential risks and complications for my pet. You are to use all reasonable precaution against injury, escape, or death of my pet. The clinic and staff will NOT be held liable for any problems that develop provided reasonable care and precautions are followed.
I understand that ANY problem that develops with my pet while I am absent will be treated as deemed best by the staff veterinarians and I assume full responsibility for the treatment expense involved.
I understand that a deposit may be required prior to services being rendered on my pet. If I neglect to pick up my pet within 5 days of the date below and do not notify you within that time frame you may assume that my pet is abandoned and are hereby authorized to dispose of my pet as you deem best and / or necessary.
